Psychological (mental) status Pupils: size, symmetry, reaction Paired ocular movements Papilloedema
Pressure (BP, increased ICP) Pulse and rate
Paralysis, Paresis
Pyramidal signs
Pin prick sensory response
Pee (incontinent)
Patellar relex (and others)
Ptosis

Reevaluate patient every 8 hrs.
